Medical Info

Introduction:
We need to collect medical information on all competitors in the Games.  We have produced two forms. Please ensure that you complete the correct forms!
All people registering for the Games (competitors AND supporters) MUST complete a waiver Form.

Haemodialysis Information Form:
This form must be completed by ALL participants requesting Haemodialysis services during the week of the Games.  We will base our bookings for haemodialysis services on the forms we receive back.  Participants must complete the forms and return them to their Team Managers As Soon As Possible. The Team Manager must return them to the LOC in Dublin by June 4^th at the latest.

We will only send forms requesting test results and other information to people who complete and return the Haemodialysis Information Form on time.  These forms will be sent out by June 11^th at the latest.

Medical Form Transplant & Peritoneal Dialysis:
This Form is to be completed by ALL competitors who are transplant recipients or are on Peritoneal Dialysis.  Please ensure that all the relevant sections are completed and that your Consultant Physician signs the form and enters their comments.  This form must be returned to your Team Manager As Soon As Possible.  The Team Manager must return the forms to the LOC in Dublin by post to arrive not later than July 8^th.

Waiver:
The Waiver Form must be completed by ALL participants in the Games (Competitors AND Supporters).  The forms must be returned to your Team Manager As Soon As Possible.  The Team Manager must return the Waiver Forms to the LOC in Dublin by post to arrive not later than July 8^th.
